Tennesseeans traveling to NY that don’t leave contact information could face $2,000 fine and court order
If you’re a Tennessee resident planning to fly to New York and you fail to leave your contact information with the department of health, you could face up to a $2,000 fine.
Governor Andrew Cuomo and the NY Department of Health have announced expansion to the travel advisory list, which now includes 22 states including TN.
The following states are now on the list:

If you're traveling to NY from the following states you must self-quarantine for 14 days:
The states are: AL, AR, AZ, CA, FL, GA, IA, ID, KS, LA, MN, MS, NC, NM, NV, OH, OK, SC, TN, TX, UT, WI.

Anyone from the listed states must quarantine for 14 days when entering NY, and is required to fill out a form ensuring they follow the mandatory quarantine. Several of the listed states are seeing significant rises in coronavirus cases. In addition to a fine, those who do not leave information may also be subject to a court order mandating the following of state quarantine requirements.
For people traveling by other means of transportation like car or bus, they must fill out the form online.
